Permanent LicenseWelcome to Maine. We are pleased you've chosen to apply for a license to practice medicine here. Maine is now participating in the FSMB Uniform Application process. This means that there is now one place to apply for your Maine Permanent Medical License as well as a application. Maine requires that you use the Federation Credentials Verification Service (FCVS). Please review the State of Maine Requirements for Medical Licensure (PDF) before you start the application process. Then click on the "Start Here" link below to start your FCVS application and/or your Uniform Application process.
Exam InformationIf you are applying for a license to practice medicine in Maine, there is a written test given as part of the licensure process. The exam review materials can be seen by clicking here (PDF) and we encourage you to download and review them prior to taking the test. Exam Review Materials (PDF). Online Exam – Effective in 2011, The Board has directed that all renewing licensees take and pass an open book exam on the Board's licensing rules and policy statements. The exam is online at http://www.docboard.org/me/exam . Upon its completion, the results will be immediately available to you. The exam must be taken every four years. Online Renewal for existing Permanant LicensesMDs with existing licenses that are due for renewal can now renew their license online by going to:www.maine.gov/online/doclicensing [NOTE: Please remember that your license number has been converted from the old format of 012345 to the new format of MD12345 and you will need to use the newly formatted number to log on to the online licensing system.]Applications Below you can find applications for temporary and emergency licenses, applications for reinstatement, renewal, and for camp licenses and renewals. Important Credit Card Instructions: If you choose to pay your fee with a credit card, you must provide the following information: Type of Card (MasterCard or Visa only!): Card Number: Expiration Date: Amount: Credit Card Holder name and billing address as it appears on the card: This information can be included with your form on a separate typewritten sheet and mailed or faxed to the Board or you can phone it in. Temporary LicensePlease note the following information. The fee for a temporary license is $300, payable to "Maine Board of Licensure in Medicine". Any physician who is qualified under 32 MRSA, section 3275 may, without examination, be granted a temporary license for a period of time up to 6 months. That license may be extended for up to 6 months, but may not exceed one year, when the board deems it necessary to provide relief for local or national emergencies or for situations in which there are insufficient physicians to supply adequate medical services, including Locum Tenens needs. Emergency (100 Day) LicenseA physician who presents a full, current, active, unconditioned license from another U.S. licensing jurisdiction and who can provide reasonable proof of meeting qualifications for licensure in Maine may, without examination, be granted a temporary license for a period not to exceed 100 days, when the board deems it necessary to provide relief for declared local emergencies or for other appropriate reasons as determined by the Board. The fee for this temporary license shall be $300, payable at the time of application. Administrative License"A License Limited to the Practice of Administrative medicine" means: A. professional managerial or administrative activities related to the practice of medicine or to the delivery of health care services, but does not include the practice of clinical medicine; and/or, B. medical research (excluding clinical trials on humans). If the physician’s job does not include any act as defined in this rule as “clinical medicine”, a License Limited to the practice of Administrative Medicine is required. Camp License or RenewalPlease download the appropriate form and follow the attached instructions. Volunteer Conversion or Volunteer Renewal The physician has retired or is retiring from the active practice of medicine and wishes to donate his or her expertise for the medical care and treatment of indigent and needy patients in the clinic setting of clinics organized, in whole or in part, for the delivery of health care services without charge. Educational CertificatesEducational Certificates are used to apply to practice in a residency program ReinstatementYou may apply for a reinstatement if your license has lapsed or if you have withdrawn your license within the last five years. Please download the appropriate form and follow the attached instructions. After withdrawal of your Maine license is approved, to be eligible to reinstate your license you will have to provide evidence of having actively engaged in the practice of medicine for the 12 months preceding the reinstatement application, or you will have to demonstrate current competency by passing a written examination or other practical demonstration as the Board may prescribe. You will also have to provide evidence of having achieved at least 40 category I CME credits and at least 60 Category II credits in the prior 24 months. Verifications to other State BoardsWe are now participating in the "VeriDoc" electronic license verification system. This system is intended for use by physicians who practice in the United States and need to have verification of the status of their active and inactive medical licenses sent to another state medical board. The Board of Licensure in Medicine sends VeriDoc our database of licensees on a daily basis. Maine-licensed MD's who are applying for licensure elsewhere will apply for Maine verifications to be sent to that licensing jurisdiction by logging on to www.veridoc.org .
