BADEEN, Steve J.; 05/12/1992; Conditional license issued by Consent Agreement. Been treated since 1987 for chronic, habitual, intemperate use of alcohol.

BRIGGS, Linwood; 08/12/1992; Received letter of Warning and $100 fine. Violation of Medical Practice Act and Board Rule for failure to respond to consumer complaint received by the Board and for failure to respond to the Board's request to meet with Dr. at an Informal Conference.

HORNSTEIN, Louis S.; 12/09/1992; Adjudicatory Hearing was to be held regarding charges of unprofessional conduct and incompetence. Dr. failed to show at Hearing.

HORNSTEIN, Louis S.; 11/10/1992; Emergency 30 day suspension. Dr. requested continuance of Adjudicatory Hearing. Given nature of allegations of unprofessional conduct and incompetence, Board voted to issue emergency suspension pending Hearing.
