BEVERLY, Bert T., M.D.; 02/18/1993; Voluntary surrender of medical license. Surrendered during Board investigation of allegations of substance abuse.

DAS, Satyendra N., M/D. 01/12/1993; License denied. License denied on basis of failure to satisfy license examination requirements.

HUNT, John B., M.D.; 10/27/1993; Through consent agreement, M.D. agreed to indefinite suspension. Entered Consent Agreement during Board investigation of allegations of substance abuse and controlled substances missing from two hospitals.

KITFIELD, Edward M.D.; Withdrawn from Consent Agreement.

LEACH, William C., M.D.; 10/27/1993; Through consent agreement M.D. agreed to a modified license, 2 reprimands and 2 $1500 civil fines. Piggyback on Florida original action of 07/16/1991. Fraud or deceit on 2 applications for licensure in Maine, a 1991 application for reinstatement and a 1992 application for renewal.

RICHARDSON, Douglas C., M.D.; 02/24/1993; Physician fined $500 and required to attend risk management seminar. Following consumer complaints, Board found inadequate or insufficient patient communication and insufficient procedures for promptly providing patient records.

SCULLION, David F., M.D.; 06/24/1993; Voluntary surrender of medical license. Surrendered during Board investigation of allegations of substance abuse and failure to comply with Federal requirements pertaining to controlled substances.

SPENCER-LeMAY, Carol, M.D.; 03/09/1993; Placed on probation; probation lifted 11/09/1993. Following a hearing, Board found M.D. committed unprofessional conduct by breaching a patient's confidentiality.

WERNER, Donald, M.D.; Consent Agreement, 9/01/93; Conditional License; Incompetence
