ADVERSE LICENSING ACTIONS - 2000BLOCH, Silvia M., M.D.: License #014372; 12/12/00. In lieu of proceeding to an Adjudicatory Hearing, the licensee agrees to accept a reprimand from the Board. In addition, the licensee agrees to attend an introductory course on neurology for psychiatrists given by the Post Graduate Medical Education Division of Harvard Medical School or an equivalent course sponsored by a medical school and approved by the Board. The Board will permit the licensee to withdraw from licensure. This action is based on unprofessional conduct. BUDD, William L., M.D.: License # 005768; 5/9/00. In lieu of proceeding to an Adjudicatory Hearing, the licensee agrees that the status of his license will be changed from Active to Inactive, effective May 9, 2000. The licensee may not practice medicine in Maine or in any other state. This action is based on unprofessional conduct and incompetence. CYR, JC (Joseph) Guynemer, M.D.: License # 004470; 7/11/00. At the conclusion of an Adjudicatory Hearing held on July 11, 2000 the Maine Board of Licensure in Medicine voted to suspend the licensee's license for 360 days and refer the matter to administrative court for revocation of his license to practice medicine. However, the licensee agreed to immediately surrender his medical license to the Board and retire from the practice of medicine effective July 11, 2000. The Board accepted the Licensee's surrender of his license. This action was based on fraud and deceit in obtaining a license, incompetence in the practice of medicine and the existence of a mental or physical condition that may endanger the health or safety of patients. HARRIS, Verne D., M.D.: License # 014445; 4/6/00. In lieu of proceeding to a hearing, the licensee agrees to accept a reprimand from the Board for unprofessional conduct based on a sexual relationship with a patient. Based on action taken by the California Medical Board. KEATING, Anthony J., M.D.: License # 006059; 5/9/00. In lieu of proceeding to an Adjudicatory Hearing, the licensee agrees to accept a warning from the Board and pay a $1,500 fine. This action is based on inappropriate prescribing practices. LUCK, Gregory C., M.D.: License # 010063; 5/4/00. Licensee has an Inactive status license subject to a Consent Agreement. The licensee has been suspended effective May 4, 2000 for violation of the monitoring requirements of the Consent Agreement entered into with the licensee on July 13, 1999. PHELPS, Robert N., Jr., M.D.: License # 009869; 6/13/00. In lieu of proceeding to an Adjudicatory Hearing, the licensee agrees that his license will be placed on probation indefinitely beginning June 13, 2000, with the following conditions: the licensee will continue in counseling at a minimum of twice a month for at least two years; the licensee agrees to have a chaperone present when seeing female patients in his office; the licensee will be subject to mandatory clinical and professional supervision by a colleague to be approved by the Board and the licensee agrees to educate himself about the ethical and boundary issues involved in the doctor-patient relationship. Any substantiated report to the Board alleging sexual impropriety or misconduct shall result in immediate, indefinite and automatic suspension of the licensee's license. This action is based on unprofessional conduct, incompetence and a violation of the Sexual Misconduct Rules of the Board, based on inappropriate relationships with two female patients. ROBERTSON, Donald M., M.D.: License # 005912; 12/12/00. In lieu of proceeding to an Adjudicatory Hearing, the licensee agrees to limit himself to practicing without a DEA registration. He previously surrendered his DEA registration and agrees not to reapply for one. The licensee agrees to random chart reviews for two years beginning on the effective date of this agreement. This action is based on inappropriate prescribing practices. SARACINO, Anthony M.D.: License # 013504; 4/15/00. In lieu of proceeding to a hearing, the licensee agrees to accept a reprimand from the Board for unprofessional conduct based on a sexual contact with a patient. The licensee will educate himself about boundary issues involved in the doctor/patient relationship. STOJANOV, Igor, M.D.: License # 014796; 6/13/00. In lieu of proceeding to an Adjudicatory Hearing the licensee agrees to accept a reprimand. The licensee will be on probation for one year beginning June 13, 2000, during which time he must obtain treatment or counseling which will increase his understanding of his inappropriate behavior, and prevent recurrence of that behavior. The treatment provider or counselor must be approved by the Board. The licensee agrees to have a chaperone present for all examinations of female patients. If the licensee relocates to another state, he must notify that state's licensing board of the terms of this agreement. This action is based on unprofessional conduct, incompetence and a violation of the Sexual Misconduct Rules of the Board based on an inappropriate relationship with a female patient. SUNSHINE, THOMAS J., M.D.: License # 013101; 09/30/00. Licensee complied with the terms of his Consent Agreement which was effective 10/01/95. Restrictions and conditions on licensee’s Maine medical license are removed effective 09/30/00. TESSIER, Paul A., M.D.: License # 006620; 1/11/00. In lieu of proceeding to a hearing, the licensee agrees to accept a reprimand and pay a $1,500 fine as discipline for unprofessional conduct. Action based on the licensee's failure to report restriction of clinical privileges at Irwin Army Community Hospital on two applications for medical staff privileges at Maine hospitals. TRUMBULL, Donald , M.D.: License #011516; 12/12/00. The licensee's license was reinstated by Consent Agreement with the following conditions: the licensee will not see, care for or treat minor female patients; the licensee agrees to continue in treatment with a therapist until the therapist and the licensee mutually agree to termination; the therapist will provide quarterly reports to the Board; the licensee agrees not to practice as a sole practitioner and the licensee must tell his partners or his employer of the terms of this Consent Agreement and the prior Consent Agreement. WOODRUFF, Alan F., M.D.: License # 006151; 08/08/00. At the conclusion of an Adjudicatory Hearing held on August 8, 2000 the Maine Board of Licensure in Medicine voted to suspend the licensee's license for 360 days and refer the matter to Administrative Court for revocation of his license to practice medicine. WOODRUFF, Alan F., M.D.: License # 006151; 07/17/00. The licensee's license to practice medicine was summarily suspended effective July 17, 2000, due to his violation of the March 3, 1998 Consent Agreement by prescribing medication for his wife or himself. WOODRUFF, Alan F., M.D.: License # 006151; 07/14/00. In lieu of proceeding to an Adjudicatory Hearing the licensee agrees to surrender his DEA registration. He will not reapply for his DEA registration until two years from the effective date of this agreement. The licensee will be on probation for two years. During this time the licensee will complete educational programs approved by the Board regarding proper prescribing of controlled substances.
