ADVERSE LICENSING ACTIONS - 2003AMFILO, BASIL, M.D.: LICENSE # 005240; 04/24/03. Licensee's Maine medical license was non-renewed effective 4/24/03. This action was based on incompetence in the practice of medicine. BARKIN, JEFFREY S., M.D.: License # 014724; 04/14/03. Licensee complied with the terms of his Consent Agreement, which was effective 04/14/98. Restrictions and conditions on licensee's Maine medical license are removed effective 4/14/03. CEDERNA, JOHN P. M.D.: License # 012732; 1/14/03. In order to avoid an adjudicatory hearing, the licensee agrees to the following: The licensee's failure to comply with his prior Consent Agreement with the Board and his failure to make appropriate arrangements when he closed his practice constitutes unprofessional conduct. The licensee agrees to accept a Reprimand as discipline. HARRISON, JOHN W., M.D.: LICENSE # 016151; 07/08/03. Effective July 8, 2003 the Board voted to amend the Consent Agreement entered into with the licensee on January 14, 2003 by discontinuing the requirement of random monthly urine monitoring. The other requirements of the Consent Agreement and the Licensee's contract with the Physicians Health Program remain in effect. HARRISON, JOHN W. M.D.: License # TD-02-128; 1/14/03. The licensee and the Board agree that the issuance of a license is conditioned upon the licensee's compliance with the following requirements: The licensee will participate in the Physicians' Health Program for a period of two (2) years and comply with whatever terms are required by the PHP contract. During the two year period, the licensee will submit to random monthly urine monitoring. This action is based on substance abuse issues. HEARN, WALTER N., P.A.-C: License # PA-060; 12/23/03. At the conclusion of an Adjudicatory Hearing held on December 9, 2003 the Maine Board of Licensure in Medicine voted to suspend the licensee's license for a period of 28 days. The licensee is credited for the 28 days during which his license was suspended while awaiting his Adjudicatory Hearing. The licensee was also reprimanded and placed on probation pursuant to the terms of the Consent Agreement, with a new effective date of December 9, 2003. This action is based on positive test results while under Consent Agreement for substance abuse monitoring. HEARN, WALTER N., P.A.-C: License # PA-060; 11/12/03. Automatic suspension for positive test result while under a Consent Agreement for substance abuse monitoring. HEARN, WALTER N., P.A.-C License # PA-060; 05/13/03. In lieu of proceeding to an Adjudicatory Hearing, the licensee agrees to a Consent Agreement in which he accepted a reprimand from the Board. In addition, the licensee will be subject to substance abuse monitoring weekly for a period of one year from the effective date of the agreement and at least monthly for the four years thereafter. HOLDREDGE, TIMOTHY S., M.D.: License # 015004; 11/12/03. Licensee complied with the terms of his Consent Agreement, which was effective 09/08/98. Restrictions and conditions on licensee's Maine medical license are removed effective 09/08/03. HOPE, PETER B., M.D.: License # 013648; 02/11/03. In order to avoid an Adjudicatory Hearing, the licensee agrees to voluntarily surrender his Maine medical license as of the effective date of this agreement. He will no longer be authorized to practice medicine or write prescriptions in the State of Maine. The voluntary surrender is permanent and the licensee will not seek reinstatement of his Maine medical license. This action is based on unprofessional conduct. HYDE, NATHANIEL W., M.D.: License # 012201; 09/10/03. In lieu of proceeding to an Adjudicatory Hearing, the licensee agrees to accept a reprimand from the Board. In addition, the licensee and a representative from the Board will arrange for a retrospective review of all emergency admissions over the past 6 months by a surgeon approved by the Board. The reviewer will submit a report in 6 months. The licensee will arrange a prospective, ongoing review of all acute abdominal cases by the same reviewing surgeon, with a report to the Board in one year. For the next license renewal period the licensee will document ongoing education in the areas of critical care and emergency care. KENDALL, KEVIN M., M.D.: License # 014562; 01/14/03. Licensee complied with the terms of his Consent Agreement, which was effective 9/30/98. Restrictions and conditions on licensee's Maine medical license removed effective 1/14/03. KESSLER, MICHAEL W. M.D.: License # 016184 ; 05/29/03. By Consent Agreement, a medical license was issued subject to the following conditions: The licensee will be subject to substance abuse monitoring at least twice monthly for a period of four (4) years from the effective date of this agreement. Following the period of four (4) years from the effective date of this agreement the Board or the Supervising Physician may request random samples at any time. This action is based on discipline by other state medical boards due to alcohol dependency issues. KLUGER, Paul B., M.D.: License # 012600; 11/12/03. Licensee complied with the terms of his Consent Agreement, which was effective 11/10/98. Restrictions and conditions on licensee's Maine medical license are removed effective 11/10/03. LENTZ, ALISON M., M.D.: LICENSE # 016087; 02/21/03. At the conclusion of an Adjudicatory Hearing held on February 11, 2003 to hear evidence regarding the Board’s preliminary denial of the licensee’s application for a permanent Maine medical license, the Board voted to grant the licensee a Maine medical license subject to the following conditions: The licensee shall receive therapy at a minimum of once per month. The therapy shall center on providing insight into the licensee’s emotional landscape and the manner in which that impacts on patient care and how patients may respond to her. The therapist shall submit a report to the Board by February 11, 2004. The Board will subsequently reconvene and review this matter. LOISELLE, ANDRE B., M.D.: License # 011663; 06/08/03. Six month suspension of Maine medical license ended, effective 6/8/03. The licensee's Maine medical license remains subject to the conditions of the Consent Agreement which became effective on January 8, 2002. MCGILL, THOMAS, M.D. LICENSE # 013991; 05/01/03. Licensee complied with the terms of his Consent Agreement, which was effective 05/02/98. Restrictions and conditions on licensee's Maine medical license are removed effective 05/01/03. MORONG, DOUGLAS M., P.A.-C: LICENSE # PA-503; 03/11/03. In lieu of proceeding to an Adjudicatory Hearing, the Licensee agrees that his conduct with respect to signing Dr. Thomas K. Murray's name to a plan of supervision without Dr. Murray's permission constitutes unprofessional conduct. The Licensee agrees to accept a reprimand as discipline for his action. NICHOLSON, ANDREW S. M.D.: License # 015480; 1/14/03. In lieu of proceeding to an Adjudicatory Hearing and in order to resolve an outstanding complaint, the licensee agrees to the following discipline: The licensee will restrict his practice of medicine to adults over 18 years of age. The licensee will participate in ongoing treatment with a therapist, approved by the Board, with experience in the treatment of sexual offenses. The Board will receive quarterly reports from the therapist.The licensee will be on probation and the Board will order a reevaluation of the licensee after one year from the effective date of this agreement. This action is based on unprofessional conduct.
O'KEEFE, GREGORY M.D.: License # 008247; 05/29/03. At the conclusion of an Adjudicatory Hearing held on May 13, 2003, the Board voted to grant the applicant's appeal of the Board's preliminary denial of license renewal and to issue the licensee a conditional license to practice medicine in Maine. The conditions of licensure relate to type of practice and requirements for peer review and random chart review. RODRIGUEZ, MANUEL, M.D.: LICENSE # 008639; 09/09/03. In lieu of proceeding to an Adjudicatory Hearing the licensee agrees to voluntarily surrender his medical license as of the date of this agreement and agrees not to seek reinstatement of his license to practice medicine in Maine. ROSENBLATT, STANLEY, M.D.: LICENSE # 005552; 06/10/03. In lieu of proceeding to an Adjudicatory Hearing, the licensee agrees to voluntarily surrender his Maine medical license as of the date of this agreement and agrees not to seek to renew his license to practice medicine in Maine. This action is based on unprofessional conduct, incompetence and inappropriate prescribing practices. ROSENBLATT, STANLEY, M.D.: License # 005552; 04/15/03. Summary suspension of license due to unprofessional conduct, incompetence and inappropriate prescribing of controlled substances which may be an imminent danger to the public. SCHRADER, JOSEPH B. P.A.-C: License # PA-369; 01/14/03. Licensee complied with the terms of his Consent Agreement, which was effective 2/10/98. Restrictions and conditions on licensee's Maine physician's assistant license are removed effective 1/14/03. STEINBERG, IRA, B., M.D.; License # 014272; 09/09/2003. Granted reinstatement of an Inactive License. STURDEVANT, DAVID F., M.D.: LICENSE # 012561; 09/09/03. In lieu of proceeding to an Adjudicatory Hearing the licensee agrees to engage a professional office management company to review coding and documentation practices. In addition, the licensee will educate himself about the management of chronic pain patients, including the proper prescribing of controlled substances, the characteristics of drug seekers and the recognition of drug diversion. The licensee will document his ongoing education in these matters in his next two applications for renewal of his medical license. VIOLETTE, E. JEFFREY, M.D.: License # 012065; 06/30/03. By Consent Agreement the licensee's medical license is reinstated to Active status subject to the following conditions: the licensee will be subject to substance abuse monitoring monthly for a period of five (5) years from the effective date of this agreement. Following the period of five (5) years from the effective date of this agreement the Board or the Supervising Physician may request random samples at any time. WILLSON, SHAWN L., M.D.: License #016223; 07/08/03. By Consent Agreement the licensee is issued a license subject to the following conditions: the licensee will be subject to substance abuse monitoring at least once each month for a period of four (4) years from the effective date of this agreement. Following the period of four (4) years from the effective date of this agreement the Board or the Supervising Physician may request random samples at any time. WILSON, JAMES E., P.A.-C: License # PA-684; 11/12/03. Automatic suspension for positive test result while under a Consent Agreement for substance abuse monitoring. WOODRUFF, ALAN F., M.D.: License # 006151; 06/30/03. By Consent Agreement the licensee's medical license is reinstated subject to the following conditions: the licensee agrees never to prescribe for himself or a family member and the licensee agrees that violation of this agreement will result in the automatic, indefinite suspension and possible revocation of his medical license.
